Every expression of human endeavour, whether it be music, art, architecture, mathematics, agriculture or medicine, is structured in the eternal laws of nature which govern and order our existence.\u003c\/p\u003e\n\u003cp\u003eAs a child growing up in India, I had a traditional education and was exposed to many aspects of these Vedic sciences, but the area that really captured my imagination was health. From a young age, I was fascinated by the human body. I wanted to see inside it and find out how it worked. I would often sneak into hospitals and, from hidden vantage points, watch the doctors perform surgery. I also realized, even as a boy, that I had a strong desire to ease the suffering of others.\u003c\/p\u003e\n\u003cp\u003eSeeing my preoccupation with health, my father told me the story of my great, great grandfather, Shankarji Joshi. Shankarji was a famous vaidya or Ayurvedic physician, renowned throughout India for his skill in this ancient healing science. His entire life was dedicated to bringing health to the people of his area, and he did so until he passed on at the age of ninety-six. It was said that by the time he was sixty-eight, he had lost many of his teeth and his vision had weakened. He went away for a period and underwent kaya kalpa, an ancient rejuvenation procedure documented by the Ayurvedic texts, which not only restored his vitali-77 and eyesight, but also gave him back his teeth.\u003c\/p\u003e\n\u003cp\u003eMy father once said that at the time of Shankarji's death, he told his son - my father's father- that he would return to bring the knowledge of Ayurveda to his people, and that he would come back as the son of Vinayak, the name given to my dear father. Whether I am, in fact, that great soul, Shankarji, is of little consequence. It is, however, his inspiration burning inside me that guided me to the medical profession. I did not immediately gravitate toward my great-grandfather's calling as a vaidya.\u003c\/p\u003e\n\u003cp\u003eI entered a school that offered training in both Western (allopathic) and Ayurvedic medicine and had to decide between them. Both my parents worked hard as educators but were not well paid for their efforts. As the eldest son, I wanted to make their life comfortable and financially secure. Western medicine promised greater financial security, adding this incentive to my long fascination with surgery Around the time that I received my medical degree in Ayurveda my mother became acutely ill. For years she had suffered from amoebiasis, a parasitic infection common throughout India. Treatment by allopathic doctors had only temporarily alleviated her symptoms. Finally, the amoebas had invaded her liver and formed cysts, causing her to enter a life-threatening phase of the disease called amoebic hepatitis. She was admitted to the hospital for intensive care. However, as a result of being treated with powerful but toxic drugs, she began to experience a range of new symptoms, including further metabolic breakdown, leg pain, and headaches. She was so depressed that she begged us to remove her from the hospital. Feeling confused and in fear for her life, we brought her home.\u003c\/p\u003e\n\u003cp\u003eA family friend suggested that we take her to a local Ayurvedic practitioner. We saw this as a last resort, but we had nowhere else to turn. After a thorough examination, Dr. Katti, the Ayurvedic physician, gave her some herbal preparations and strict dietary guidelines. In three weeks she had completely recovered. Now, many years later, my mother maintains good health and still takes care of her family.\u003c\/p\u003e\n\u003cp\u003eI was deeply affected when I saw my mother respond so quickly to such a simple treatment program after years of suffering through unsuccessful allopathic therapies. My father reminded me that this ancient system of health care was my heritage, handed down to me from my great, great grandfather. I began to reconsider my direction in medicine. Now that I had my M.D. in Ayurveda, I had to select an area of medicine for postgraduate study and specialization. Until this point, I was committed to pursuing modern medicine, but I started having second thoughts about Western medicine. I was aware of its many achievements but was also painfully familiar with its shortcomings, especially the deleterious side effects of many of its therapies and the high cost to patients and their families.\u003c\/p\u003e\n\u003cp\u003eAn inner voice encouraging me to investigate Ayurveda grew stronger in my heart and I decided to study it in depth at the All India Research Institute in Nagpur, a city in central India. I already knew about some of the significant differences between Ayurveda and the Western approach to diagnosis and treatment. However, as I probed more deeply into Ayurvedic theory and clinical practice, I became impressed by its logic and charmed by the power and completeness of its comprehensive. Understanding of life. It described a vital connection between individual life and the whole of nature; human life was perceived as a microcosm or intimate reflection of the entire universe. As I continued my studies, I became fascinated by the area of Ayurvedic science known as Panchakarma, a comprehensive system of knowledge and practices to purify the body of toxins and restore it to balance with natural law. In case after case, I watched Pnchakarma achieve complete remission of disease in patients which could not be helped by Western medicine. In addition, I saw how this science of purification and rejuvenation could take a person with relatively normal health and produce a dramatically greater state of health, happiness and fulfilment. In fact, Ayurveda aims to establish the ability to live every aspect of life to its fullest, in complete conscious connection to nature's infinite intelligence, a state often referred to as enlightenment.\u003c\/p\u003e\n\u003cp\u003eWhen I first arrived in America, I wondered how different the health concerns would be from those of my country. Aside from the higher incidence of drug and alcohol addiction, psychological imbalance and emotional abuse that seemed to be typical of the West, the problems I began to see were remarkably similar. It seems that no country has been able to escape the negative impact of technological life. The increasingly fast pace of modern life, the growing environmental degradation and the loss of connection to the natural rhythms of life are taking their toll on health and happiness everywhere. As more and more people are falling prey to the adverse effects of stress, I hear more frequent complaints of disturbed sleep, chronic indigestion, poor elimination and lower back pain. Lack of physical and sexual vitality, chronic fatigue, allergies and the inability to either lose or gain weight is also very common problems.\u003c\/p\u003e\n\u003cp\u003ePerhaps the most serious side effect of this growing health crisis in the world is the unhappiness that I witness on the faces of the people who come to see me. So many express strong dissatisfaction with their jobs or relationships and seem to have lost their motivation and enthusiasm for life. As a consequence, anxiety and depression have become all too common. Almost everyone I see has expressed some feeling of frustration with the healthcare systems that are available, invariably remarking that they have tried everything and nothing seems to work. One system might address their complaint as a structural misalignment, another as a biochemical imbalance, and still another as a blockage in the flow of energy. The famous physician Jivaka, known from Buddhist sources, is said to have studied medicine at the university in Taxila, where some sources name his teacher Atreya, describing him as a surgeon. The Atreya Punarvasu, who taught medicine to Caraka and other founders of ayurveda, was probably different from Jivaka’s teacher but nevertheless is also sometimes described as teaching medicine in Gandhara on one occasion. From this, I always wanted to tell a comprehensive background story collaborating fully with the archaeological assessment of the surgical and medical artefacts unearthed by the archaeologists in the light of their expert appraisal.\u003c\/p\u003e\n\u003cp style=\"text-align: left;\"\u003eThirdly, being an anaesthetist (anaesthesiologist) for 40 odd years and watching surgical procedures closely I had observed the improvements in the outcome within my practising life were so visible, which I always attributed to unremitting advance and improvements of the available tools, a process that can be traced to the time of Celsus or even before, The evolutionary enhancements in instrumentation helped to improve the skills of both the surgeons and anaesthetists. A new blood pressure monitor or a modified surgical clamp, a hip implant made from a better metal or an inert plastic intraocular lens, all such new innovations consistently produced much better results than the previous versions. The opportunity of working in operating theatres, anaesthesia and intensive care surrounded by today’s rapidly changing technology was a great help and inspiration. My fascination for instrumentation made me an avid collector of medical antiques and induced a passionate interest in the evolution of surgical instruments. My curiosity about the surgical instruments discovered from the sites of Gandhara was a natural upshot of my hobby.\u003c\/p\u003e\n\u003cp style=\"text-align: left;\"\u003eThe excavated objects are silent witnesses, though they cannot speak but can make credible eyewitnesses if suitably analysed. The method employed in the study is a comparison with known instruments from the Greek and Roman periods. Such an approach is reflected in the selection of the pictures where an effort is made to highlight by comparison the fact that in general design, size, shape and function of most of the instruments, equipment and pharmaceutical-related objects excavated in Gandhara do bear a resemblance with the acknowledged Roman or Greek objects despite the fact that their design was independently evolved by the people living far away from each other. Such a comparative study is intended to bring about an interesting emphasis for the reader who can see the essential details in two different objects at a glance and then makes an unbiased opinion regarding the argument. In search of surgical instruments of the period I have visited most museums in Pakistan, including the National Museum at Delhi and in order to look at the real Roman and Greek surgical instruments I have also travelled to the National Archaeological Museum at Naples and the Museum at Epidaurus in Greece. While relevant information from some others such as the Museum of Calcutta, Madras Museum (still known by old names), and Allahabad Museum was obtained by correspondence.\u003c\/p\u003e\n\u003cp style=\"text-align: left;\"\u003eThe consequence of the study of surgical tools evidently ricochets onto the social history of the blacksmiths who made them, doctors or surgeons who used them and the patients who might have benefited or suffered from them. Even though the geography of the region contains the most natural borders in the form of the highest mountains in the World and the great rivers; this should have created lasting geographical boundaries. But the area always remained and still is the most porous, its inhabitants have always moved freely without any restrictions. From the second millennium BCE till the present this rugged region on both sides of some of the highest mountains in the World and their snow-covered peaks remained a melting pot where a number of diverse civilisations arrived from far away lands and left their marks which can be seen and felt even today. When it came under the Buddhist influence that lasted for many centuries it achieved a high point in learning both in arts and sciences. During this period Gandhara seems to be unique among the contemporary civilisations to independently develop its own medicine and more importantly practice surgery based on rational principles.\u003c\/p\u003e\n\u003cp style=\"text-align: left;\"\u003eIt is a fact of history and archaeological reality that the Buddhist Gandhara was the only place in the World outside the Roman Empire where high surgical skills were acquired and put into practice. The history of art, literature and various sciences taught in the universities of Gandhara has been well documented. However (the historiographers have not fully examined or talked about the available evidence that supports the advancement which was achieved in medicine particularly, the corroborated evidence regarding surgery despite unequivocal archaeological verification. In this study, an attempt is made to investigate the archaeological and historical evidence and evaluate how surgery and medicine developed, progressed and then declined in Gandhara. It is important to keep in mind that the history of medicine of any civilisation cannot be studied in isolation, since it is directly influenced by the ongoing social changes, cultural conflicts and upheavals and most importantly by the technological progress that the society can manage. Here we must also look at the technological progression of the people of Gandhara that enabled them to attain the advance in rational medicine and surgery during the Buddhist domination. In the chronology of events inclusion of the relevant social wax and wane and their consequences are not only inevitable but an overriding factor for better orientation of the course of history. We are told that the major changes which took place in the development of brain size and intelligence coincided with the ability of human beings to live together in social groups.’ Glancing through the evolution of medicine we are further informed by medical anthropologists that the newly socialised human beings as a matter of necessity started practising some rudimentary form of medicine and even surgery at quite an early stage. The medicine man seems to be present among the earliest settlements. The anthropologists have also made a case for caring among early societies from the discovery and study of toothless bones belonging to individuals who were incapable of biting or chewing and must have been provided suitably prepared food by others within the community. Such communal responsibilities and interdependence within the family unit offered the opportunities for man to learn how to protect the frail or weaker family members from many of the hazards faced by the primitive tribal communities. In the hostile surroundings, senior and wiser clan members assumed the guardianship of the needy by protecting and looking after the very young, very old and very weak, nursing and feeling the injured and those who were ill. According to Sir William Osler (1849-1919), ‘medicine arose out of the primal sympathy of a man with man, out of a desire to help those in sorrow, need and sicknesses’. He describes the whole of yoga in total of 195 aphorisms, divided into four parts. He defines yoga as chittavritti nirodhah and then writes remaining aphorisms to explain how to achieve it. He considers yoga as a discipline. Unless yoga is practised regularly, sincerely and in a most disciplined manner, one can not reap its benefits. The first four steps are known as bahirang yoga (the external part of yoga) and the rest four steps form antarang yoga (the internal part of yoga) or raj yoga. Patanjali does not mention the names of asanas or pranayamas but elucidates the fathoms of human psychology and recommends prophylactic measures to promote and maintain physical and mental health.\u003c\/p\u003e\n\u003cp style=\"text-align: left;\"\u003eAhimsa (non-violence), satya (truthfulness), asteya (non-stealing), brahmacharya (abstinence), and aparigrah (non-possessiveness) are five yamas or the vows of self-restraint (the don’ts) that are meant for one’s virtuous attitude towards the society. Purification of self is necessary before proceeding to yoga practices or simultaneously, to reap the full benefits of yoga. Such purification is achieved by observing these rules (commandments) of Patanjali. It helps to understand the mechanism of yogic practices and their appropriate application for the patient or a spiritual aspirant or the normal individual.
